Rules for Power of Attorney

• Eligible documents for apostille/authentication in Utah

  Utah will authenticate original or certified-copy public documents (vital records, court/county records, school documents, business filings) and properly notarized documents (including POAs, adoption papers, school transcripts, contracts).

• Private documents (like a POA) require proper Utah notarization

  The POA must be notarized by a Utah-commissioned Notary Public, with a valid notarial certificate, signature, seal, and all usual notarial formalities.

• Only original or certified copies accepted — no scans or uncertified copies

  Photocopies, scanned copies, or documents issued outside Utah or by federal agencies are not eligible for apostille/authentication.

• Apostille vs Certificate depends on destination country

  For Hague-Convention countries, Utah issues an Apostille; for non-Hague countries, a Certification of Authentication is issued instead.

• Standard apostille/authentication fee and processing time

  Regular service fee is $19 per document; processing takes about 3–5 business days (not including mailing).

• Submission requirements for apostille/authentication

  Must submit the original or certified document, a completed request form, payment, and a self-addressed return envelope or prepaid shipping label.

• Office location and submission methods

  Documents can be mailed to or dropped off at the Lieutenant Governor’s office in Salt Lake City (350 North State Street, Suite 220) or submitted during walk-in hours or by appointment.

• Apostille/authentication certifies only the signature/ seal, not the content

  The apostille confirms the authenticity of the signature and seal or official status, but does not certify the truth or content of the document.
