Must Be a Certified Copy
You must submit an original certified copy issued by the Utah Office of Vital Records and Statistics (Utah Department of Health and Human Services). It must have the official seal and the signature of the State Registrar.
Certificate Must Be Recent
Utah enforces a strict recency requirement. The certification date on the vital record must be within one year of the date it is submitted to the Utah Lieutenant Governor's Office for the apostille.
Do Not Notarize the Certificate
The certified death certificate itself should not be notarized. The apostille is applied based on the signature of the State Registrar. Notarizing it is an incorrect step that will cause rejection.
Complete the Request Package
