  • Include Payment and Return Envelope

    Submit the correct fee (check or money order payable to "Tennessee Secretary of State") and a self-addressed, stamped envelope for the return of your document.

  • No Notarization Required (Do Not Do It)

    You should NOT have your certified Tennessee birth certificate notarized. The apostille is placed directly on the certified vital record itself, authenticating the signature of the State Registrar. Notarizing it will cause rejection.
