Must Be a Certified Copy from the Court
You must submit an "Original Certified Copy" issued by the Prothonotary or Clerk of Court in the Pennsylvania county where the divorce was granted. A notarized photocopy is not acceptable.
Proper Signature and Seal
The document must be signed by the Prothonotary, Clerk of Courts, or a deputy, and must bear the official seal of that county's court.
Document Must Be Complete and Unaltered
The certified copy must be in pristine condition with no tears, staple holes, or any alterations. If it is a multi-page document, all pages must be certified as one complete document by the court.
Complete the Request Form
Pennsylvania requires a completed "Authentication/Apostille Request Form" (PDF). This form is mandatory and must accompany your request.
Correct Payment
The fee is $15.00 for the first apostille and $5.00 for each additional apostille submitted at the same time. Payment must be made by check or money order payable to the "Commonwealth of Pennsylvania". Cash is not accepted. Include a self-addressed, stamped envelope for return shipping.
