Certified Document

Complete Cover Sheet
You must include a completed "Notary Authentication/Apostille Request Form" which acts as your cover sheet. This form is available on the Ohio Secretary of State's website and is mandatory.
Correct Payment
Include a $5.00 per apostille fee. Payment must be made by check or money order payable to "Ohio Secretary of State." Cash is not accepted for mail-in requests.
