    The certified copy must be in pristine condition with no tears, staple holes, or any alterations. If it is a multi-page document, all pages must be certified as one complete document.

  • Complete the Request Form

    New York requires a completed "Authentication Request Form" (PDF-1). This form is mandatory and must accompany your request.

  • Correct Payment

    The fee is $10.00 per apostille. Payment must be made by check or money order payable to the "New York Department of State". Cash is not accepted. Include a self-addressed, stamped envelope for return shipping.
