Must Be a Certified Copy from the Court
You must submit an "Original Certified Copy" issued by the County Clerk's office of the New York Supreme Court (or the court that granted the divorce) in the county where the divorce was finalized. The certification must be an original.

The certified copy must have the original signature of the County Clerk (or a Deputy County Clerk) and must bear the official seal of the County Clerk's office or the court.
Document Must Be Complete and Unaltered
The certified copy must be in pristine condition with no tears, staple holes, or any alterations. If it is a multi-page document, all pages must be certified as one complete document.
Complete the Request Form
New York requires a completed "Authentication Request Form" (PDF-1). This form is mandatory and must accompany your request.
Correct Payment
The fee is $10.00 per apostille. Payment must be made by check or money order payable to the "New York Department of State". Cash is not accepted. Include a self-addressed, stamped envelope for return shipping.
