Rules for Marriage Certificate
Certified Document
You must submit an original certified copy issued by the Nevada Office of Vital Records (Department of Health and Human Services) or a County Recorder/County Clerk's office. It must have the original signature of the State Registrar or County Official.
County Clerk/Recorder Certification (Critical Step)
Before sending it to the state, the certified marriage certificate must be taken to the County Clerk's office in the same county that issued it. The County Clerk/Recorder will complete a "Certificate of Authentication" to verify the signature on the vital record. This certificate is attached to the document. Rejection Cause: Sending the document without this county certification. The state will not apostille the document without it.
Complete Cover Sheet
You must include a completed "Document Authentication/Apostille Request Form" which acts as your cover sheet. This form is available on the Nevada Secretary of State's website and is mandatory.
Correct Payment
Include a $20.00 per apostille fee. Payment must be made by check or money order payable to "Secretary of State." Cash is not accepted for mail-in requests.
