Rules for Marriage Certificate

  • Certified Document

    You must submit an original certified copy issued by an Illinois County Clerk's office (where the license was obtained) or the Illinois Department of Public Health (Vital Records). It must have the original signature of the County Clerk or State Registrar and a raised seal.

  • County Clerk Certification (Critical Step)

    Before sending it to the state, the certified marriage certificate must be taken to the County Clerk's office in the same county that issued it. The County Clerk will complete a "Certificate of County Clerk" form to verify the signature of the official who signed the marriage certificate. This form is attached to the document. Rejection Cause: Sending the document without this county certification. The state will not apostille the document without it.

  • Complete Cover Sheet

    You must include a completed "Authentication Request Form" which acts as your cover sheet. This form is available on the Illinois Secretary of State's website and is mandatory.

  • Correct Payment

    Include a $2.00 per apostille fee. Payment must be made by check or money order payable to "Illinois Secretary of State." Cash is not accepted for mail-in requests.

  • Pre-Paid Return Shipping

    You must include a self-addressed, stamped envelope with sufficient postage. Using a trackable service like USPS Priority Mail is highly recommended.
