Must Be a Certified Copy from the Court
You must submit an "Original Certified Copy" issued by the Circuit Court Clerk in the Illinois county where the divorce was granted. A notarized photocopy is not acceptable.
Proper Signature and Seal
The document must be signed by the Circuit Court Clerk (or a Deputy Clerk) and must bear the official seal of that Circuit Court.
Document Must Be Complete and Unaltered
The certified copy must be in pristine condition with no tears, staple holes, or any alterations. If it is a multi-page document, all pages must be certified as one complete document by the court.
Complete the Authentication Request Form
Illinois requires a completed "Authentication Request Form" to be submitted with your request. This form is mandatory.
Correct Payment
The fee is $2.00 per apostille. Payment must be made by check or money order payable to the "Illinois Secretary of State". Cash is not accepted for mailed requests. Include a self-addressed, stamped envelope for return shipping.
