Certified Document

Complete Request Form
You must include a completed "Document Authentication Request Form" which acts as your cover sheet. This form is available on the Hawaii Lieutenant Governor's website.
Correct Payment
Include a $1.00 per document authentication fee. Payment must be made by check or money order payable to "State of Hawaii." Cash is not accepted for mail-in requests.
