Rules for Death Certificate
Must Be a Certified Copy
You must submit an original certified copy issued by the Hawaii State Department of Health, Vital Records System. It must have the official seal and the signature of the State Registrar. Photocopies are not acceptable.
Certificate Must Be Recent
Hawaii has a specific recency requirement. The certification date on the vital record must be within one year of the date it is submitted to the Hawaii Lieutenant Governor's office for the apostille.
Do Not Notarize the Certificate
The certified death certificate itself should not be notarized. The apostille is applied based on the signature of the State Registrar. Notarizing it is an incorrect step that will cause rejection.
Complete the Request Package
Ensure your submission includes all required elements: The original, recent, certified death certificate. Important Note
