Certified Document
You must submit an original certified copy issued by the Georgia Department of Public Health (State Office of Vital Records) or a Georgia County Probate Court. It must have the original signature of the State Registrar or Judge/Probate Court Clerk.
Notarization is REQUIRED (Critical Step)
This is Georgia's key requirement. The certified marriage certificate must be notarized by a Georgia Notary Public before you send it to the Secretary of State. The notary must affix their seal and complete their notarial certificate. Rejection Cause: Sending the document without this notarization. The state apostilles the notary's signature, not the Vital Records official's signature directly.
Complete Cover Sheet
You must include a completed "Authentication Request Form" which acts as your cover sheet. This form is available on the Georgia Secretary of State's website and is mandatory.
Correct Payment
Include a $3.00 per apostille fee. Payment must be made by check or money order payable to "Georgia Secretary of State." Cash is not accepted for mail-in requests.
