    The certified copy must be in its original condition. Do not staple, tape, or hole-punch the document. The raised seal and signature must be completely visible and intact.

  • Complete the Request Form

    You must include a completed "Apostille / Authentication Request Form" with your submission. This form provides your contact information and details for the apostille.

  • Include Payment and Return Shipping
