Must Be a Certified Copy from the Court
You must submit an "Original Certified Copy" issued by the Clerk of the Circuit Court in the Florida county where the divorce was granted. A notarized photocopy is not acceptable.
Proper Signature and Seal
The document must be signed by the Clerk of the Circuit Court (or a Deputy Clerk) and must bear the official seal of that court. The signature must be an original; a stamped or printed signature is often insufficient unless it is officially placed there by the clerk's office.
Document Must Be Complete and Unaltered
The certified copy must be in pristine condition with no tears, staple holes, or any handwritten alterations. If the decree is multiple pages, all pages must be stapled together and certified as one complete document by the court.
Complete the Cover Sheet
Florida requires a completed "Apostille or Authentication Request Form" (Form DOS-1397). This form is mandatory and must accompany your request.
Correct Payment
The fee is $10.00 per apostille, plus a $1.00 convenience fee per document if using the paper form. Payment must be made by check or money order payable to the "Florida Department of State". Include a self-addressed, stamped envelope for return.
