Rules for Death Certificate
Must Be a Certified Copy
You must submit an original certified copy issued by the Florida Bureau of Vital Statistics or a Florida County Health Department. It must have the official raised seal and the signature of the State Registrar or Local Registrar. Photocopies are rejected.
Certificate Must Be Recent
Florida has a firm recency requirement. The certification date on the vital record must be within one year of the date it is submitted to the Florida Department of State for the apostille.
Do Not Notarize the Certificate
The certified death certificate itself should not be notarized. The apostille is applied based on the signature of the Vital Records official. Notarizing it is an unnecessary and incorrect step that will cause rejection.
Complete the Request Package
Ensure your submission includes all required elements: The original, recent, certified death certificate. A completed "Apostille Authentication Request Form" (available on the FL DOS website).
