  • Document Requirements for a Power of Attorney

    Your POA must be an original or a certified copy, and: Must be notarized by a Connecticut Notary Public, or Certified by a qualifying Connecticut official (Town Clerk, Registrar of Vital Statistics, Court Clerk, Justice of the Peace, etc.) Additional requirements: The POA must be notarized or certified within the last 10 years. It must contain an original signature and/or official seal. Documents signed by out-of-state notaries cannot be authenticated. Note: Federal documents and out-of-state vital records must be authenticated by the U.S. Department of State or the issuing state — Connecticut will not process them.

  • What You Must Mail

  • Return Shipping

    If you include a prepaid return label, Connecticut will return your POA using that method (FedEx, UPS, DHL, USPS). If you do not include prepaid shipping, your POA will be returned via first-class mail. International returns require your own prepaid label.

  • Copies of POA

    A copy of a Power of Attorney can be authenticated, but only if: You attach a written statement affirming the copy is true and accurate; This statement is sworn and signed before a Connecticut Notary Public.
