Use the Correct Document
You must submit a "Certified Copy" issued by the Connecticut Department of Public Health (DPH), Vital Records Unit, or a Connecticut town vital records office. It must have the original raised seal and signature from the Registrar.
No Notarization Required (Do Not Do It)
You should NOT have your certified Connecticut birth certificate notarized. The apostille is placed directly on the certified vital record itself, authenticating the signature of the Registrar. Notarizing it will cause rejection.
Ensure the Document is Original and Unaltered
The certified copy must be in its original condition. Do not staple, tape, or hole-punch the document. The seal and signature must be completely visible.
Complete the Request Form
You must include a completed "Document Authentication Request Form" with your submission. This form provides your contact information and return address.
Include Payment and Return Envelope
