Rules for Death Certificate
Must Be a Certified Copy
You must submit an original certified copy issued by the Arizona Office of Vital Records (or a local Arizona vital records office). It must have the official seal and signature of the State Registrar. Photocopies or notarized copies of a certificate are rejected.
Certificate Must Be Recent
This is a critical rule in Arizona. The certification date on the vital record must be within one year of the date it is submitted for the apostille. This is non-negotiable.
Do Not Notarize the Certificate
