Rules for Death Certificate
Must Be an Original Certified Copy
You must submit the original death certificate issued by the Alaska Bureau of Vital Statistics, with a raised or stamped seal. Do not send a photocopy or the funeral home's copy.
Certificate Must Be Recent
The certification date on the vital record must be within one year of the date you submit it for the apostille. Older certificates will be rejected.
Do Not Notarize the Death Certificate
The certified death certificate itself should not be notarized. It is certified by the state, which is sufficient. Notarizing it will likely cause a rejection.
Complete the Request Package
Include all required items: The original, recent, certified death certificate. A completed Document Authentication/Apostille Request Cover Sheet. The correct fee (currently $2.00 per apostille). A self-addressed, stamped envelope for return shipping.
